[xml] 2 issues

1st one:

I've just upgraded to v2-2.2.2 from v2-2.0.0 and a change that's been made to
xmlDocDump is causing me problems. It now closes the supplied file where it
didn't used to.

Any idea how I can work around this? I guess if xmlDocContentDumpOutput wasn't
static I could roll my own xmlDocDump. I'm surprised this hasn't affected
anyone else really!

2nd one:

Ideally I'd like to stop xmlDocDump (well, xmlDocContentDumpOutput)
automatically adding the XML version tag to the start of the output. It causes
an error in the 3rd party component I'm sending the output to. I can't change
the 3rd party component so I'm kinda stuck.
